Dr David Atwood, keynote speaker at the Community Frailty Audit workshop

Wessex AHSN Healthy Ageing Programme warmly welcomed Dr David Atwood (GP Partner and GP with Special Interest in Older People at Pathfields Medical Group/Clinical Lead for Integrated Care of Older People, Devon and Honorary Secretary for the British Geriatrics Society) as their keynote speaker at the Community Frailty Audit workshop on Thursday 13th February 2020 at the Axis Conference Centre, Southampton. The event was well attended by system leaders from across the health, social care and the voluntary sector, where the output from the Wessex Community Frailty was shared and the delegate was challenged to identify the what opportunities are there across Wessex to support the PCN frailty agenda and how can we foster joint relationships between acute, primary care, social care, community, voluntary services and PCNs? The video clip is 45 minutes in length, but is a fantastic resource for GP and PCN colleagues developing the case for population health management for older people – it also provides a great framework for discussion for CCGs, social care and voluntary service in how joined up working can improve care, reduce demand and provide cost savings. It also provides real life approaches to overcoming information sharing across systems and showcases the development of a comprehensive geriatric assessment template within an GP settings – inspirational and innovative work!
