Project focus area
The focus of the national Cardiovascular Disease (CVD) programme in 2025/26 is heart failure. Our vision is to improve outcomes (increase quality of life, reduce admissions and re-admissions and improve prognosis) for people living with heart failure through supporting pathway transformation and clinical servicer redesign using innovative diagnostic tools, pharmacological interventions and remote monitoring solutions.
Following detect, protect and perfect methodology our vision is to improve outcomes (increase quality of life, reduce admissions and re-admissions and improve prognosis) for people living with heart failure through supporting pathway transformation and clinical servicer redesign using innovative diagnostic tools, pharmacological interventions and remote monitoring solutions. This aligns with the government’s three big shifts - hospital to community; treatment to prevention; and analogue to digital.
We also support local systems with their ongoing prioritisation of CVD prevention and work in collaboration with our regional health innovation network collaboratives in the South East and South West, delivering educational webinars, a quarterly CVD newsletter, sharing resources and aligning with regional priorities.
Alongside this work, our 2025-26 InHIP programme is focused on optimising the lipid management of secondary prevention patients in primary care, to enable them to achieve NICE-recommended targets, significantly reducing the potential for additional events such as heart attacks or strokes.
