How and when people are discharged from hospital matters. Discharging people too early or without the necessary support can be unsafe and increase the risk of readmission. Delayed discharge can increase the risk of hospital-acquired infection, and is associated with low mood and reduced motivation, which can also increase the risk of readmission. For older people in particular, long stays in hospital can lead to loss of muscle strength – by up to 5–10% within the first seven days – which can hinder their return to living independently or increase the level of support they need. 

Health Innovation Wessex is working collaboratively with the Hampshire and Isle of Wight ICB and system including NHS providers, social care, industry, innovators and researchers, to help identify innovative AI and digital solutions to support and improve discharge and flow. We will be exploring cultural aspects of adopting new ways of working and successful conditions providing knowledge mobilisation and ensure learning and recommendations are shared openly and transparently for patients and the workforce alike.  

This programme underpins the three national government shifts and the 10 Year Health Plan supporting digital & AI transformation, organisational change and supporting workforce development, leadership and culture whilst complementing HIOW ICB’s frailty strategic agenda.
