Project
As part of the Wessex-wide Ageing Well programme 2026 -2027, we are working closely with Dorset integrated care board partners, provider organisations, and community partners to find, test, implement and scale innovation. Our vision is to see innovation supporting healthier ageing and reducing avoidable pressure on health and care services across the region.
Together with our partners and building on innovation scanning work completed in 2025 -2026, we have identified four areas to focus on and explore the innovations that might improve patient experience and outcomes.
1. Community Rehabilitation:
Working collaboratively with both clinical teams and innovators we will build on previous innovation scanning work. We are currently exploring innovation to support digital platforms that deliver personalised programmes to help prevent falls.
2. Workforce:
Exploring innovative ways of engaging with the student workforce, assisting with workforce planning and new ways of working by:
• Working with Bournemouth University to develop a new way of working with student physiotherapists to deliver strength-based activities for people at risk of falling.
• Working with the Pan Dorset Stroke service to embed an innovation to improve planning, productivity and delivery of neighbourhood and out of hospital care.
3. Dementia and Social connection:
Scoping work with local acute and neighbourhood leads on the challenges, priorities and opportunities for innovation adoption for those living with dementia, their carers and older adults living with loneliness who don’t have meaningful social support networks.
4. Perioperative Pathways:
Working with system partners to explore innovations across the planned surgery pathway for those living with multiple long-term conditions from assessment, through waiting well to post operative support.
