Health Systems Support Framework: Support for NHS organisations moving to integrated models of care

The Southern AHSNs (Kent Surrey Sussex, South West, Wessex, and West of England) have been accredited to provide services on NHS England’s new Health Systems Support Framework.

 These services are:

• Transformation and change support
  
• Patient empowerment and activation

What is the Health Systems Support (HSS) Framework?
The Health Systems Support (HSS) Framework provides a quick and easy route for any NHS organisation to access support services from innovative third party suppliers at the leading edge of health and care system reform, including advanced analytics, population health management, digital and service transformation.

The Framework contains pre-negotiated standard terms and conditions to which the suppliers have signed up. Find out more here.

Who can use the Framework?
The Framework can be used by NHS trusts and other care providers, GP, primary care and community providers, local authorities, Integrated Care Systems (ICSs) and Sustainability and Transformation Partnerships (STPs), Clinical Commissioning Groups (CCGs), and a wide range of public sector bodies where they are supporting STPs, ICSs, or health and social care in general. What support are the Southern AHSNs providing as part of the HSS Framework?
The HSS Framework is divided into 10 lots. The Southern AHSNs are accredited to provide services in lots 6 and 7:

Lot 6: Transformation and change support 

• patient pathway optimisation and care model design
  
• specialist advice on organisational redesign, governance and payment and contract reform
  
• communications and engagement
  
• workforce and leadership development support
  
• scaling innovation
  
• primary care at scale support.

Lot 7: Patient empowerment and activation

• Support for implementing shared decision making: advisory support to implement tools and training to facilitate shared decision making and support clinical culture change.
  
• Support for implementing self-care programmes (including social prescribing and innovative digital and remote technologies and e-consultations): linked to new or existing platforms that have the capability to collate data across the system and analyse it to inform care provision and improve patient outcomes across a population.
• Services include but are not limited to:
  
• identification of patients for intervention
  
• advisory services participation
  
• implementation services
  
• patient portals
  
• support for implementing Personal Health Budgets and Integrated Personalised Commissioning support
  
• IT solutions to support integrated personal commissioning and Personal Health Budgets
  
• care planning
  
• system redesign
