Wessex Patient Safety Collaborative – Learning Event 1

3 August 2015

Wessex Patient Safety Collaborative (PSC) supports staff working in practice across Wessex to deliver the safest care possible, through a collaborative structure where organisations, networks and teams can learn from each other and from experts in topic areas.

Collaborative teams from Wessex organisations participate throughout the year attending 4 learning events and then testing improvements in practice using Plan-Do-Study-Act [PDSA] cycles in between events.

Copies of the materials used during the first Learning Event can be found on this page. Any enquiries about these materials should be made to the Patient Safety Team at the Wessex Academic Health Science Network (AHSN) via email at patientsafety@wessexahsn.net or by telephone on 023 8202 0844.

You can also follow us on Twitter @WessexPSC and use the hashtag #saferwessex.

First Learning Event Monday May 11th to Tuesday 12th May 2015

The aims of this learning event were to:

  • Explore the Institute for Healthcare Improvement [IHI] Breakthrough Series methodology
  • Deliver sessions from experts on sepsis, transfers of care, measurement and leadership
  • Allow space for teams to plan their local patient safety improvement PDSA cycles
  • Support staff to undertake PDSA cycles of improvement, measurement and monthly reporting
  • Facilitate networking and shared learning across teams
  • Ensure co-design and co-production with patients.

Our Storify page shows how the event progressed.
