Join us for the fourth instalment of the Wessex Innovation Forum where we will be focusing on one of the most pressing challenges in health and social care - discharging patients home. This is a great opportunity to make connections with others working in this space locally and explore the potential of innovation to support a joined up approach to hospital discharge and community-based care. It can also count towards your self-directed CPD hours. 

Wessex continues to face major challenges discharging patients home, reflected in national data which shows a yearly increase in delayed discharge and 14,000 patients a week in January 2025 who no longer meet the criteria to reside. These delays reduce available beds, intensify pressure on urgent and emergency care, and expose patients to avoidable risks such as de-conditioning, infection, and longer recovery times.

Innovative approaches and digital tools hold the key to increasing capacity while reducing inefficiencies. However, this problem can't be solved in isolation, and for the benefits of innovation to be realised there needs to be a collaborative approach across the NHS, local authorities, and care providers. This event aims to enable collaboration between these groups, and will explore innovations and approaches to improve patient experience, reduce the need for formal care services, and ultimately facilitate a smoother patient flow at scale.
