CVD Central - Offer from KSS AHSN

Are you offering Blood Pressure and / or Pulse checks to people in any setting?

If so, you can pick and choose from the 5 different options outlined in the CVD Central slide set and use whatever suits and supports your project best.

The CVD Central project is delivered in collaboration by KSS AHSN, British Heart Foundation and NHS Benchmarking, to help with understanding and demonstrating the impact and outcomes from health checks of various types in a variety of clinic and community settings.

Options 1-4 on offer for any organisation and setting to utilise:

• 1. = Results Cards

• 2. = 1^st patient experience survey – on the day of check
• 3. = 2^nd patient experience survey – one month post check
• 4. = Resource pack – available in Word format and anyone is welcome to download amend / adapt it to suit your local project work… please feel free to add to it, re-brand it, or just take whatever you want from it to add to your own project.

and

• 5. For PCNs only - Clinical activity and outcome data collection, this will be managed by providing you with an EMIS /TPP search designed to extract BP data, captured through events.

Please do share the CVD Central slide set across your networks /  teams and encourage use of the generic patient experience survey/s in particular as we are super excited to start seeing the data flow in at scale!
