Introduction
We have completed working across Hampshire, Dorset, IOW and South Wiltshire to spread this initiative. The timing and detail of the process involved in becoming a DF surgery differs from surgery to surgery, but there are some basic steps that are undertaken. These involve staff training, patient and carer engagement, links to the voluntary sector in the locality, an environmental audit, dementia-focussed care planning, and completion of tools (such as ‘This is Me’) to support the person with dementia and their carer. This data comprises the final monthly report for Wessex CCGs, setting out the surgeries that have completed the steps to become dementia friendly (DF) and those in the process of completing this work. This work is being mainstreamed to Wessex CCGs early in July 2017 and the final report of the project has been widely shared.

NB There is a minor erratum at the start of the talk. The three delivery models in Nottinghamshire relate to the spread of a dementia friendly toolkit developed during the iSPACE pilot in Derbyshire.
