Map of Medicine – Applying the PAIN approach to problem solving

An End of Life Care project in Dorset

The End of Life Care Project was delivered in 2013 with Dorset County Hospital NHS Foundation Trust & the Weldmar Hospicecare Trust after the outgoing South West SHA identified that acute trusts had room for improvement in their end of life care.

DCHFT and Weldmar delivered a project around: 1) multi-professional communication skills training in 12 workshops for all clinicians working in the trust, and 2) the establishment of an End of Life Care Champions Group for registered and non-registered healthcare workers in the trust to attend study days and learning & practice days to see EOL care taking place in another setting.

Documentation Improvement Project: Sandra Walker

The Documentation Improvement Project was established to improve the level of communication and the standards of documentation in a mental health rehab unit, using the NMC Guidelines for Record Keeping and self-audit tools to drive out adverse effects on patient care.

Wessex Patient Safety Collaborative Launch & Listen event November 2014

The Wessex Patient Safety Collaborative welcomed patient representatives, patient safety professionals and stakeholders to its inaugural Launch & Listen event held at the Ageas Bowl, Southampton, on Tuesday 11th November 2014.

Wessex AHSN is leading and facilitating a programme of patient safety improvement work across the region, working closely with a wide range of stakeholders across health, social, judicial and wider public sectors. We will assist organisations to identify where quality improvement is needed and help them to make patient care safer.
