The CVD PREVENT audit is part of a broader strategic objective outlined in the Long Term Plan to prevent 150,000 strokes, heart attacks and cases of dementia over the next ten years. The audit prioritises working with system partners to drive CVD quality improvement at individual GP, PCN, and ICS level. The Annual Audit Reports and Data & Improvement Tool supports quality improvement in primary care for the prevention of CVD in England by helping organisations to identify variation, trends, and opportunities in the prevention and management of CVD conditions.
Hampshire and Isle of Wight work to date
The CVD Lead Pharmacist team have been using educational outreach and CVDPREVENT to change healthcare professional behaviour in Primary Care and facilitate CVD Management Recovery across Hampshire and the Isle of Wight. This has taken the form of informal sessions that include looking at relevant CVDPREVENT audit data and specific practice data, using this to prioritise work then forming a plan based on Quality Improvement principles.
