The digital primary care activities at Wessex AHSN aim to support strategic goals aligning to regional and national priorities. The Self Care Technology (SCT) project is an initiative between Wessex AHSN and Hampshire and the Isle of Wight ICS. The aim of the project is to discover the self care technology needs in primary care, run demonstrator sites of potential solutions and scale up those with the most demonstrable benefits and impact. The project is funded by HIOW ICS.
In early 2021 primary care colleagues who were interested in The SCT project took part in a rapid insight event where with clinicians and health professionals discussed the opportunities for supporting patients to self manage their conditions through digitally enabled services. Key condition areas participants wanted to focus on were weight management, diabetes and hypertension. Following this, NHS England (NHSE) released the National Digital Weight Management Programme which supports adults living with obesity who also have a diagnosis of diabetes, hypertension or both, to manage their weight and improve their health.
