Developing New Care Models (NCM) initiative to improve stroke services in Wessex: conceptualisation of an evaluative framework.

This poster presents the conceptualisation of an evaluation of the development of a new care models initiative to improve stroke services in Wessex. Using mixed methods, thematic insights were provided into the development of the NCM initiative. Examples of such themes included improved relations with colleagues and working towards a patient-centred approach.
