Isle of Wight Integrated Localities Evaluation Report

This 2019 set of summative slides looks at the implementation journey of the integration of health and social care services into locality hubs on the Isle of Wight. This is an extension of the Local Area Coordinators evaluation which conducted in 2018. The findings look at both reactive case review and the start of risk stratification work for the most vulnerable patients.
