Independent Evaluation of the North East Hampshire and Farnham Making Connections Service

This 2018 evaluation report follows the expansion of the Making Connections Social Prescribing service in Farnham, to the other 4 localities in North East Hampshire, using Vanguard funding. The evaluation found good evidence that the service makes a positive impact on the lives of its clients and how they feel about the important things in life.
