Independent evaluation of the North East Hampshire and Farnham Vanguard Referral Management Service

This report evaluates the implementation of a peer review referral management service in North East Hampshire & Farnham in 2017. The report looks at the impact of the service in redirecting referrals to appropriate community services, in improving the quality of referrals from general practice and the benefits for patients of these changes.
