Happy, Healthy, at Home North East Hampshire and Farnham Vanguard 2015-2018 Summative Evaluation Report

This is a summative report based on the evaluations of the new models of care implemented as part of the Vanguard programme in North East Hampshire & Farnham (2015-2018). The report assesses delivery against the 3 key aims of improving support to keep people well, providing joined up & accessible local care and providing specialist care in the community where needed. The report highlights the impacts of the changes on both patients and staff, describes the active ingredients for success and reflects on the timescales for seeing system level change.
