Independent evaluation of Care Navigators on the Isle of Wight

This 2018 report looks at the introduction of Care Navigator roles as part of the Isle of Wight’s Vanguard Programme. These new roles work alongside the community and voluntary sector to provide support to those over 50, through a social prescribing model. The report highlights the challenges faced in setting up the service and evidences some of the early impacts seen.
